Chronic Care Management (CCM) is the non-face-to-face clinical time spent on medicare beneficiaries with two or more chronic conditions. CMS started reimbursing physicians for CCM in 2015, and they have drastically increased the reimbursements for CCM. As of January 1, 2020, CMS has five reimbursements for Chronic Care Management.
